Will My Health Insurance Cover This Service?

Foundations Developmental House is contracted and in-network with Blue Cross Blue Shield and Aetna plans. All other carriers are considered out-of-network.

For those families with Blue Cross Blue Shield or Aetna, our Billing Specialist will call to check the benefits of your particular plan (as a courtesy) and bill out your child’s therapy session claims, however this is not a guarantee of coverage or reimbursement. Not all services are covered by all insurance policies. Whereas FDH is unable to obtain exact benefit details, we will attempt to provide you with the most accurate information regarding the specifics of your plan and what your financial responsibility will be.

We strongly encourage you to call your insurance company directly to ask questions and familiarize yourself with your plan details. It is important to know if there is a deductible or copay, what your coinsurance is (if applicable), and what your out-of-pocket maximum is. These things will determine how much you will pay for a service. Also, some plans may require a Dr’s referral or script before services can be rendered. Other plans may require a pre-authorization for certain services. Please be aware of any possible restrictions or limitations your benefit plan may have, such as the maximum number of sessions allowed per calendar year, the frequency and duration of services allowable, and if your plan limits or denies therapy services for certain diagnostic codes.

Your insurance plan will have the final determination in how a claim is processed.

Clients are responsible to pay FDH, LLC directly for services rendered. Payment is due at the time of service for all appointments. FDH reserves the right to discontinue services in the event that families do not adhere to payment agreements and policies.

Private Pay Service Cost

The private pay cost for an initial speech evaluation is $950.00 and for occupational therapy is $450.00. The evaluation cost comprises the testing time with your child, scoring and interpretation, report writing, and face-to-face consultation time to review findings and discuss treatment and/or program recommendations.
